Your First Visit
On a first visit to Shen Tov Wellness Center, we will spend approximately one hour with our patients obtaining a complete health history. Question coverage will include current health, past health history, family health history and social history (exercise, diet, smoking etc.). Depending upon the results of the health history, an examination will also be performed during the initial visit. The examination will consist of vital signs, height, weight, neurological and orthopedic examinations, muscle strength testing, postural/gait analysis and range of motion testing. Additional tests, when applicable, may include blood and or urinalysis. Patients may be referred to an appropriate facility for advanced imaging or other services.
After the health history and examination, each patient will receive a detailed explanation of all findings and our assessment. A treatment plan including short and long term goals will be discussed along with the expectations of the physician and patient. Treatments may consist of, but are not limited to, spinal manipulation, acupuncture, therapeutic modalities and nutritional counseling. Appropriate referrals will be made should the initial treatment conclude that the patient’s needs are best served through the services of another physician.
IMPORTANT
To help us serve you more efficiently, please completely fill out the intake forms prior to your appointment to allow more time for the examination and treatment of your health. It is recommended to bring any medical charts, radiology reports and/or lab results in your possession prior to the first appointment. Medical charts may be faxed over from your physician/clinic’s office to (773) 529-4200. Spouse’s or partners are welcome to join our patients during their first appointment.
Cancellation Policy
Cancellation policy — A 24 hour cancellation policy is in effect for all appointments. A $50 charge will be charged to your credit card on file for appointments that are missed or not cancelled with a 24 hours notice.

Payment Policy
Blue Cross Blue Shield or United Health Care co-payments are due upon completion of treatment. Balance amounts will be submitted to the insurance carrier on behalf of the patient.
Services not covered through Blue Cross Blue Shield or United Health Care will need to be paid in full at the conclusion of each treatment session by Cash or Credit. |
